You can always contact us in regards to your medical record. We keep your medical records in accordance with applicable Colorado State Law. If you need help finding a new physical therapist, please first contact your referring provider for their recommended provider list. We happy to assist you further upon request and as well, can speak with your new physical therapist to coordinate your transfer of your care including your progress and medical record information.
