Concierge Home Care Evaluation
On your first Concierge Home Care Physical Therapy visit, a complete evaluation will be performed. We will discuss your complaints, review your past medical history, medications, goals and learn about your lifestyle and function. Your evaluation will follow with a complete assessment of posture, ROM, flexibility, strength, balance, functional mobility, footwear, sensation and reflexes. A home assessment will also be performed to evaluate the safety of your home and falls risks. For examples throw rugs can be a safety hazard for falls. We will assess both the inside and outside of your home and make recommendations to improve your safety and ease of access. We will address issues including your functional mobility to perform your activities of daily living safely. We will also evaluate any durable medical equipment that you have or make recommendations based on our assessment. This may include the need for tub benches, grab bars, canes, walkers etc.. Once the assessment is complete we will discuss our findings, answer any questions, discuss a treatment plan, co-create goals and set a recommended appointment schedule tailored to maximize your progress. At each visit we will monitor your blood pressure, respiratory rate and heart rate, before and after exercise. If appropriate, you will be instructed on how to perform these measures to increase your safety and comfort with exercising independently. A variety of home care physical therapy treatments are available to help regain your strength and quality of life.
